Update docs:

- Some comments in hip_hcc.cpp
 - document hipHostRegister* flags.
 - expand docs on hipHostRegister/Unregister. Use "register" rather than
   "pin" to describe action these take.
 - change required CUDA version to 6 (require unified memory)
 - remake doxygen.
