libhsakmt: Support contiguous VRAM allocation flag

Add HsaMemFlags Contiguous bit for hsaKmtAllocMemory to allocate
contiguous VRAM, to support RDMA device with limited scatter-gather
ability.

Check KFD ioctl minor version >= 17.
