This removes almost all uses of the deprecated
__AMDGCN_WAVEFRONT_SIZE macro, which is unavailable
when targeting SPIR-V, and adds a SPIR-V compatible
formulation of warpSize (which should end up as the
sole definition of warpSize once we remove support
for treating it as a compile time constant). It
is incomplete in that the cooperative_groups
implementation will need additional surgery.

- Removed unnecessary hipEventRecord and fixed time calculation in
hipPerfDispatchSpeed.cc where it was off by a factor of 1,000.
