David Yat Sin 3c0af843e3 rocr: Remove scratch_backing_memory_byte_size
scratch_backing_memory_byte_size was originally removed, and then put
back in 02b38d0614. This was because it
was used by rocgdb. rocgdb code has been updated to not use this field.
Bumped _amdgpu_r_debug for the ABI change.

David Yat Sin 02b38d0614 rocr: Put back scratch_backing_memory_byte_size
The scratch_backing_memory_byte_size is not used by CP, but it is
currently used by rocgdb. Putting the field back, but we need to find a
solution for alt_scratch_backing_memory_byte_size.

Also, completely disabling alternate scratch as we need some changes to
support debugger.

David Yat Sin aa2f98e6f9 rocr: Update for new async scratch reclaim
Updating ROCr code to match new handshake protocol with CP FW for
asynchronous scratch reclaim.
Increase previous limits when scratch reclaim feature is available.

David Yat Sin 0344c8c0b6 Update queue structure to support async reclaim
Update queue structure to add members required for asynchronous reclaim
mechanism and dual-scratch. CP will set the AMD_QUEUE_CAPS_ASYNC_RECLAIM
bit on queue-connect to indicate whether the new features are supported.

The new members are ignored by previous versions of CP FW

Jay Cornwall f8d0ccd159 Support wave32/wave64 scratch allocations on gfx10
- Use new buffer resource descriptor layout
- Handle wave32 scratch allocation error from CP
- Make wavefront size a property of scratch allocation requests
- Repurpose wave64-specific amd_queue_t.scratch_workitem_byte_size field
- Clear index_stride field in V# on gfx10, calculated per-dispatch by CP

Qingchuan Shi ce6aee01ed Add APIs to support debugging vm fault
1. Add hsa ext api hsa_amd_register_vmfault_handler for debugger to register callback in case of VM fault.
2. Extend hsa_ven_amd_loader API to:
   (1) iterate loaded code objects in executable:
       hsa_ven_amd_loader_executable_iterate_loaded_code_objects
   (2) get loaded code object info:
       hsa_ven_amd_loader_loaded_code_object_get_info
3. Make the id of hsa_queue the same as the one used in communication with thunk (for amd_aql_queue)
